2. Select Application

Any Illinois licensed doctor, physician assistant, and advanced practice nurse is allowed to certify patients looking to to get a medical cannabis card. If your physician is not willing to certify please search our directory of compassionate physicians that would.

You no longer need to have a your certifying physician fill out paper work for you to upload. Certifications are now submitted online-only via the Illinois Cannabis Tracking System. Your certifying health care professional must create an account. Upon registration, the health care professional can log in and navigate within the platform to complete the certification of their patients.

  • Qualifying applicants can choose three caregivers
  • Caregivers must be at least a 21-year-old Illinois resident
  • Caregiver prompt will appear for patients during registration
  • Patients under the age of 18 can qualify for medical cannabis
  • Minors must have an adult caregiver legal guardian to apply on their behalf
  • Minors are only allowed to purchase cannabis-infused non-inhalation products.
  • Minors must be certified by two physicians

  • Patients who are diagnosed with a terminal illness qualify for a free medical cannabis card
  • Terminal illness is considered a diagnosis with a life expectancy of six months or less

  • Do not need a physician to complete an online certification
  • Veterans must submit a copy of their DD214 (notarized) to receive a 50% reduced application fee
  • Provide a copy of their medical records from the VA facility in the last 12 months
