Schedule an appointment with us at 888-372-0325 or schedule online to start your evaluation process! We will answer any questions you have about the evaluation process. We will also explain the need for your medical records documenting a qualifying condition and if necessary, obtain the records for you after you sign a medical release form. When you arrive for your appointment, our staff will provide you with our intake forms. Once completed, they then introduce you to the doctor. The physician will review your records, perform an exam, discuss your condition and history, and answer any questions you have regarding medical cannabis. If you have a qualifying condition the doctor will approve your certification.
The morning after your application has been submitted, the Illinois Department of Public Health (IDPH) will then upload your provisional access letter, which we will download and email directly to you. Once received, this letter along with your ID will allow you to purchase medical cannabis at the licensed Illinois dispensary you selected during the application process. The provisional access letter is valid for six (6) months and should be used until your medical cannabis card arrives in the mail. You may switch dispensaries at any time, however, you must always be registered and purchase from a medical dispensary. Medical cards are not honored at recreational dispensaries.
Upon processing and approval of your submitted application, the state Department of Health (DOH) will then send out your Medical Cannabis card, which you will receive by mail. Once received, this will allow you to continue purchase medical cannabis at the licensed Illinois dispensary you selected, this card will replace your provisional access letter.
